"This safety form should be read and signed by any and all students, LA's, or TA's working in the EECS Department Teaching Laboratories, including but not limited to 38-500, 38-600, 34-501. When signed, the completed last page of the form should be turned in to John Sweeney at the 38-500 equipment desk. No student should begin activities in the EECS Department Teaching Laboratories without first reading and completing this form. Department Faculty and Instructors are expected to make certain that students using the Department Teaching Laboratories have read and completed this form."
